The reception staff will send a proforma to your mobile phone, for you to complete. I

It is important that you complete all sections.

Your PPS no or GMS no. is needed for us to submit a COVID referral, on your behalf.

Once the form is completed one of the clinical staff will phone you to discuss your issues.

You have been in contact with a case of COVID-19

Or have returned from the UK or South Africa, or have had foreign travel within the last 14 days.
