Chronic Disease Management

The HSE has established the Chronic Disease Management Programme for Medical Card  patients and GP visit card patients , with certain underlying medical conditions.

The following chronic diseases covered are:

• Diabetes Type 2
• Asthma
• Chronic Obstructive Pulmonary Disease (COPD)
• Cardiovascular Disease including:
  • Heart Failure
  • Ischaemic Heart Disease
  • Cerebrovascular Disease (Stroke/ Transient Ischemic Attack (TIA))
  • Atrial Fibrillation

As part of the structured CDM programme, you will provide two scheduled reviews to patients in a 12-month period

It will include twice-yearly scheduled reviews with the Practice Nurse and GP, including annual blood tests and a review.
